Independent Contractor Paperwork

There are several forms you need to complete before you can start working with Access Simplified as an independent contractor (IC or 1099). Please review the brief summaries of each form below. A list of items needed to complete each form is included in the summaries. We also request that you fill out a Google Form providing your contact information to us. When you are ready, you may begin the signature process using DocuSign.

 

IRS Form W-9

If you do not complete a Form W-9 and give it to us, we will be unable to pay you until we have your W-9 on file.

What is needed to complete this form:

  • Your social security number (SSN)

 Access Simplified’s Non-disclosure Agreement (NDA)

All work you do for us remains confidential. All information you learn about us remains confidential.

 Direct Deposit (DD)

Access Simplified provides payment via direct deposit.
Please provide your direct deposit information in order to be paid.

What is needed to complete this form:

  • Your bank account number
  • Your bank’s routing number
  • Copy of your voided check OR
  • Copy of your bank statement
