Welcome to Access Simplified! As part of your hiring process, there are several forms you need to fill out. 
Please review the brief summaries of each form below. A list of items needed to complete each form is included in the summaries.

Please use your personal contact information (phone, address and email, etc.) on all paperwork unless notified otherwise.

 IRS Form W-4

(The video is for the IRS Form W-9 but the concept is similar.)
If you do not complete a Form W-4 and give it to us, we will be unable to pay you until we have your W-4 on file.

What is needed to complete this form:

  • Your social security number (SSN)
  • Last year’s tax return may come in handy

All work you do for us remains confidential. All information you learn about us remains confidential.

Acceptable Usage Policy (AUP)

This policy defines the terms of your use of Company Technology Resources (computer, internet, etc.)
You agree to act appropriately online while working. You agree to take care of any devices issued to you. 

 Direct Deposit (DD) Authorization

Access Simplified provides payment via direct deposit.
Please provide your direct deposit information in order to be paid.

What is needed to complete this form:

  • Your bank account number
  • Your bank’s routing number
  • Copy of your voided check OR
  • Copy of your bank statement

Employee Identification

You need to provide us with copies of your driver’s license and social security card.

 What is needed to complete this form:
  • Your driver’s license
  • Your social security card
